Check out our glossary of Saved Filter terms below, and learn how to properly filter patients to successfully coordinate their care:
1. Current Status: Filter patients based on their most recent event. Options include:
- Presented – patient presented to the ED
- Admitted – the patient is currently admitted
- Discharged – the patient was discharged
- Deceased – patient expired
- Closed – the patient was at a Hospital (ED or Observation), but no additional information was provided
- Medical LOA– the patient is on a Medical Leave of Absence
- Visit – the patient's most recent Ping was for an urgent care event
- No Current Status – the patient does not have any events in PatientPing
2. Facility Name: Filter patients by the name of the facility where they had their most recent event.
3. Facility Type: Filter patients by the type of facility where they had their most recent event (e.g., hospital, skilled nursing facility).
4. Setting: Filter patients by their level of care/ setting (e.g., inpatient, observation, emergency, skilled).
5. Diagnoses: Filter patients by the diagnoses that apply to their most recent care event
6. Admitted From: Filters patients by the type of the facility that they were admitted from (e.g., admitted from an ambulance). This information can be found in the patient’s encounter history.
7. Discharged To: Filter patients by their discharge disposition (e.g., discharged to home with home health). This information can be found in the patient’s encounter history.
8. Currently Billing: Filter patients by the insurer being billed for the event. Options include:
- Medicare
- Medicaid
- Commercial
- Self Pay
- None Provided
9. Event Date: Filter patients by the date of the event on their patient card. You can filter patient events by how many days or weekdays ago the event occurred.
10. Visit Duration: Filter patients by the visit duration for their most recent care event
11. Pings Flags: By using the PatientPing Flags Filter, you will be able to monitor patients who match the following flag criteria:
- High Utilizer: Patients who have presented to the Emergency Department 3 or more times within the last 60 days.
- Readmission Risk: Patients who have been discharged from an inpatient hospital stay within the last 30 days and have come back to the hospital, but have not yet been admitted.
- Recent Inpatient Stay: Patients who are currently an inpatient at a Pings hospital and who also had a prior inpatient stay within the preceding 30 days
- Recent SNF Stay: Patient who currently is at the hospital and have been discharged from skilled services at an SNF within the prior 30 days.
- 3DW: Patient whose recent SNF stay was covered under the three-day rule waiver
12. Latest Ping Resolution Status: Filter patients based on whether their Ping has been resolved or is still open. This information can be found on the Ping Feed and in the patient's encounter history.
13. Program: Filter patients by the name of the care management program(s) to which the patient is affiliated.
14. Practice: Filter your patients by the name of the practice that their attributed provider is affiliated with. This information can be found in the patient profile within the care team information.
15. Provider: Filter your patients by the name of the provider that your patients' are affiliated with. This information can be found in the patient profile within the care team information.
16. Care Coordinator: Filter your patients by the name of their care coordinator(s). This information can be found in the patient’s profile.
17. Gender: Filter patients by gender.
18. Age: Filter patients by age.
19. City: Filter patients by the city in which they live.
20. Zip Code: Filter patients by the zip code in which they live.
